Rise Health Group’s team of experienced practitioners provide comprehensive and individualised rehabilitation programs to optimise recovery and facilitate your return to work, sport and activities of everyday life.

We understand that injuries and complex medical conditions can be a significant emotional and financial burden, therefore as an approved service provider for WorkCover and the Transport Accident Condition (TAC) insurance claims, we offer ‘no gap’ rehabilitation care for all patients. Rise Health Group also accepts the Medicare benefit as full payment for services provided as a part of the Chronic Disease Management program and for all patients with a current Gold/White DVA Health Card.
Medicare

Medicare offers The Chronic Disease Management (CDM) program, formally Enhanced Primary Care or EPC, which assists in the management of patients with 'chronic and complex' medical conditions. These plans come from your GP and entitle you to a rebate of $52.95 per sessions, with a maximum of 5 sessions per calendar year. At Rise Health Group these plans are accepted by our Podiatrist, Physiotherapists and Exercise Physiologist.
CDM plans are accepted at Rise Health Group, however, all patients must have a GP Management Plan and Team Care Arrangements prepared by their GP as a part of their referral.
Assessments and follow up appointments for patients with a CDM plan are gap-free with selected practitioners. For further information, or to book an appointment please contact the clinic.
Workcover
Rise Health Group and our diverse range of services are committed to providing the highest quality of care for those recovering from a workplace injury. As specialists in the area of exercise for rehabilitation, our staff can provide you with a complete road map for your recovery and we encourage all clients to utilise our fully equipped gym and clinical Pilates studio to expedite your return to full health.

When booking an appointment which you wish to have claimed under Workcover, please inform our staff and we will record the relevant details of your case in order for us to bill directly to Workcover.
Transport Accident Commission (TAC)

Rise Health Group has a range of healthcare professionals experienced in treating patients with injuries as a result of a motor vehicle-related accident. As specialists in the area of exercise for rehabilitation, our practitioners not only have a sound understanding of how factors such as age and lifestyle impact on a patients recovery but they will also provide ongoing support through each phase of your rehabilitation program and get you moving and returning to your daily activities as soon as possible.

When booking your TAC appointment with Rise Health Group please inform the staff of your TAC claim number and case manager's details and we will discuss our billing to TAC policy.
Veterans Affairs

The Australian Department of Veteran Affairs serves veterans, war widows and widowers, serving and former Defense Force members, eligible AFP members with overseas service, and dependents and carers.
Rise Health Group works closely with the Department of Veterans' Affairs and helps support Australia’s commitment to remember and honour those who serve our nation by providing ‘no gap’ treatment for patients with a DVA GP Referral.
When booking your appointment with our staff please inform them that you are a Gold/White DVA cardholder and ensure that you bring your DVA card to your first appointment.
